I, the undersigned, hereby authorize Bare Laser & Skin Studio and its certified technicians to perform Intense Pulsed Light (IPL) treatments using DPC (Dynamic Pulse Control) technology for the purposes of:
Hair reduction
Skin rejuvenation
Reduction of pigmented lesions
Treatment of vascular lesions
Improvement of skin texture
Management of mild to moderate acne
1. Treatment Expectations
I understand that IPL/DPC treatments involve a series of sessions and multiple treatments are required to achieve desired results.
I acknowledge that results may vary depending on individual factors including skin type, hair color, hormonal conditions, and adherence to pre- and post-care instructions.
I understand that the treatment plan may be adjusted at any time based on my skin's response, and that additional treatments or skincare products may be recommended to improve outcomes or reduce the risk of complications.
2. Sun Exposure Acknowledgment
I confirm that I have avoided significant sun exposure, tanning beds, and self-tanning products for at least 2 weeks prior to my appointment.
I agree to continue avoiding direct sun exposure and use a broad-spectrum SPF 50+ sunscreen daily for a minimum of 2 weeks after each treatment.
I understand that failure to follow these guidelines may increase the risk of burns, pigmentation changes, and other side effects.
3. Risks and Possible Side Effects
I understand that while IPL/DPC treatments are generally safe, there are risks and side effects that may include, but are not limited to:
a. Redness, Swelling & Scabbing:
Temporary redness (erythema), swelling (edema), or fine scabbing may occur. This may last from a few days to several weeks. Scabs must not be picked or scratched, as this may lead to scarring or infection.
b. Hyperpigmentation / Hypopigmentation:
Changes in skin pigmentation, especially in clients with darker skin tones, may occur. These changes may be temporary or, in rare cases, permanent. Strict sun avoidance and use of SPF 50+ sunscreen is necessary before and after each session to reduce this risk.
c. Scarring & Burns:
Burns, blisters, or scarring may result from the treatment, particularly if post-care instructions are not followed or if treatment intensity must be increased to achieve results.
d. Infection:
Though rare, bacterial, viral (e.g., Herpes simplex), or fungal infections are possible. I will notify Bare Laser & Skin Studio if I have a history of cold sores in the treatment area and understand that prophylactic antiviral medication may be recommended.
e. Bleeding or Crusting:
Pinpoint bleeding or minor crusting may occur following treatment of pigmented or vascular lesions.
f. Allergic Reactions:
Local or systemic allergic reactions may occur in response to topical products, cooling gels, or aftercare creams.
g. Eye Protection:
I understand that IPL/DPC can damage the eyes and agree to wear protective eye shields throughout the entire treatment session.
4. Medical Disclosure
I affirm that I have fully disclosed my medical history, including any current medications, especially photosensitizing drugs or Accutane (Isotretinoin), both of which are contraindications to treatment.
I understand that if I am taking or begin taking such medications, I must inform Bare Laser & Skin Studio prior to my appointment.
I affirm that I have not been diagnosed with or treated for cancer in the treatment area, or if I have, I will provide medical clearance before proceeding.
5. Pregnancy, Hormones & Hair Regrowth
I understand that hormonal changes from pregnancy, childbirth, menopause, or other medical conditions may affect treatment results. IPL/DPC hair removal is not advised during pregnancy or while breastfeeding.
6. Treatment of Skin Lesions
I understand that only clearly benign lesions may be treated with IPL/DPC. Any suspicious or undiagnosed lesions must be assessed and approved by a physician prior to treatment. Once treated, the lesion may be vaporized, and laboratory testing will no longer be possible.
7. Clinic Policy & Technical Disruptions
In the rare event of a technical malfunction during treatment, my appointment may need to be interrupted and rescheduled. Bare Laser & Skin Studio will make every effort to notify me of issues in advance; however, this may not always be possible. I understand and accept this as part of my treatment plan.
8. Consent & Acknowledgment

I confirm that I have read and understood the information above.
I have had the opportunity to ask questions and all my concerns have been addressed to my satisfaction.
I accept all possible risks and side effects.
I agree to follow all pre- and post-treatment instructions provided by Bare Laser & Skin Studio.
I release the technician and Bare Laser & Skin Studio from any liability, except in the case of gross negligence or misconduct.
Cancellation & No-Show Policy Acknowledgment
Cancellation & No-Show Policy Acknowledgment To secure your appointment, a $25 deposit is required at the time of booking. This deposit is non-refundable and will be applied toward your service. We understand that life happens. However, if you cancel or reschedule within 24 hours of your appointment, you will be subject to a $25 cancellation fee. We have a strict no-show policy in place to protect our time and respect our schedule. If you fail to show up to your appointment without prior notice, you forfeit your deposit and may be required to pay 100% of the service fee before being allowed to rebook.
